29 CFR 1910.147(c)(4)(ii)(B): Energy control procedure was not developed and implemented to clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. On or about July 27, 2021 in the LyondellBasell Chemical Manufacturing Complex in La Porte, Texas, workers were exposed to process materials, toxic release, and flammable hazards when energy control procedures were not developed and implemented to block and isolate HV-117 Acetic in the Acid Unit of from hazardous energy sources while work to service HV-117 was occurring.
