29 CFR 1910.176(b):Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse. a- 380 Allwood Road, Clifton NJ: A truck driver was fatally injured when aluminum piping was stacked and during transit became displaced once unstrapped the piping struck the victim causing crushing type injuries. Condition observed on or about 07/22/2021 The employer does not ensure that all stored materials are stacked and blocked so that materials are stable and secured against sliding and displacement to prevent employee injury. This violation was observed on or about July 22, 2021, when a bundle of staged aluminum piping secured to a flatbed was released by the victim from a ratcheted load who was not protected against sliding and collapse. The victim was struck by and crushed by the descending pipes and subsequently died as a result of his injuries.
