29 CFR 1910.119(d)(3)(i)(B): The employer did not include all equipment that was part of the process on piping and instrument diagrams. On or about July 16, 2021, and at times prior thereto, for the tank farm, the piping and instrument diagram did not include all valves, overhead piping, and a blind flange used at tanks for processing flammable liquids. This exposed employees to fires and explosions.
