29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: 28 Bowditch Dr., Shrewsbury, MA: On June 19, 2021 and at times prior, the employer had not developed, documented, and ensured employees utilized an energy control procedure when they performed maintenance and servicing tasks on the BL12 Pecan Dropper machine. Employees regularly did not isolate and lock out the energy sources when they reached inside the machine to clean it out at the end of the production periods. On June 19, 2021, an employee received a fingertip amputation injury while cleaning this Pecan Dropper Machine without utilizing energy isolation procedures. Table Talk Pies was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1499032, citation number 1, item number 1a and was affirmed as a final order on May 3, 2021, with respect to a workplace located at 25 Southgate St., Worcester, MA.
