Location: 1045 Flatbush Avenue, Brooklyn, NY 11226; Roof On or about 05/17/2021 and thereafter, harnesses were not inspected prior to use. Employees wore harnesses during the demolition of a roof and were exposed to a fall of approximately 60 feet.
