29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered. On or about May 4, 2021, in the Alky 3 Unit, the employer did not ensure employees wore class C personal protective equipment when performing valve lineup to reintroduce HF acid back to the unit.
