29 CFR 1926.405(b)(2):In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy: a) Northwest corner of the break room of the office building near the the exit door to the outside: Two extension cords (to a coffee maker and a microwave oven) were plugged into a 120 volt duplex receptacle outlet; the receptacle outlet was missing the faceplate exposing the terminal screws to employee contact.
