29 CFR 1926.652(e)(1)(i): Members of support systems were not securely connected together to prevent sliding, falling, kickouts, or other predictable failure: a) North of the intersection of Piping Plover Drive and Widgeon Path, Millsboro, DE 19966: On or about 05/06/2021, the employer failed to ensure that the members of support systems were securely connected together as required. The spreader bars had obvious damage from the elongation of the spreader pin holes.
