29 CFR 1904.39(a)(1): Within eight (8) hours after the death of any employee as a result of a work-related incident, you must report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor: a) This violation occurred on or about April 22, 2021, when the employer failed to report to OSHA within eight (8) hours the death of an employee resulting from a work-related incident that occurred on April 14, 2021.
