29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: a. On or about March 3, 2021, at the above addressed jobsite, an employee performing roofing operations on a house from an ladder jack scaffold, at eighteen (18) feet five (5) inches above the ground, was exposed to fall hazards due to the lack of wearing a personal fall arrest system (PFAS). Kevin Rodriguez dba KRA Construction was previously cited for a violation of this Occupational Safety and Health Standard 1926.451(g)(1)(i), which was contained in OSHA inspection 1452699, citation 1, item number 2a and was affirmed as a final order on July 14, 2020, with respect to a workplace located at 10 Westlake Dr., South Barrington, IL 60010.
