29 CFR 1910.146(e)(1): Before entry was authorized, the employer did not document the completion of measures required by 29 CFR 1910.146(d)(3) by preparing an entry permit: a - On or about January 28, 2021, at the worksite Foundation Food Group, Inc., located at 2076 Memorial Park Drive, Gainesville, GA, at Plant 4, in the Line 2 and Line 4 liquid nitrogen freezers, employees were exposed to mechanical, thermal and asphyxiation hazards while entering permit required confined space areas within the liquid nitrogen freezers as the employer did not prepare an entry permit as required to document completion of measures taken.
