29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment. On or about January 29, 2021, and at times prior thereto, authorized employees were not issued and/or assigned a personal lockout or tagout device to be used during the servicing of the Beveller Machine, exposing employees to the hazard of being caught-in moving parts. Axis Pipe and Tube, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1910.147(f)(3)(ii)(D)), which was contained in OSHA inspection number 1425934, citation number 01, item number 002 and was affirmed as a final order on 10/31/2019, with respect to a workplace located at 1451 Louis E Mikulin Road, Bryan, TX 77807.
