29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about 1/13/2021, the employer failed to ensure that lockout/tagout procedures were followed Employees were tasked with servicing machinery including, but not limited to the Badger Foam Recycler Type FD-12 A/B, Code F012V12A2.Types of energy include electrical, mechanical, thermal, and pneumatic. Lockout/tagout procedures must be followed in accordance with 1910.147(d)(1) through 1910.147(d)(6), as follows: 1) Prepare for shutdown, 2) Machine or equipment shutdown, 3) Machine or equipment isolation, 4) Lockout/tagout device application, 5) Relieve, disconnect, restrain, and render safe all stored energy, and 6) Verification of isolation.
