29 CFR 1926.451(c)(2)(v):The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: a) At 10558 Eastern Shore Drive, Bldg. 1600 The Arlington at Eastern Shore Apartments, Spanish Fort, Alabama: On or about November 30, 2020 and at times prior; the employer failed to ensure a job made box used to transport supplies to the crew located on the roof was secured to a Powered Industrial Truck (JLG Serial number: 0160046732) which exposed employees to a struck-by hazard.
