29 CFR 1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: Entire Facility. On or about October 30, 2020 through December 17, 2020, the employer did not have an established lockout/tagout program for the control of hazardous energy during servicing and maintenance of equipment and machinery, that included the following components: 1. Documented machine-specific lockout/tagout procedures 2. Employee training for "Authorized", "Affected" and "Other" employees (with certification /documentation of training) 3. Periodic inspections (at least annual) of all procedures, including a review with each authorized employee as to their responsibilities under the procedures (with certification/documentation of the periodic inspections) Abatement documentation is required for this item in accordance with 29 CFR 1904.19(d).
