29 CFR 1910.28(b)(9)(i)(A): For fixed ladders that extend more than 24 feet (7.3 m) above a lower level, the employer did not ensure that each fixed ladder installed before November 19, 2018 is equipped with a personal fall arrest system, ladder safety system, cage, or well. On or about October 02, 2020 and at times prior thereto, at a workplace located at 2000 Robotics Place, Fort Worth, Texas; the employer did not ensure that an approximately 36 feet fixed ladder attached to the building for employee use during RTU replacement had ladder safety system and did not have fall arrest system to protect employees from falling more than 16 to 30 feet to the lower level.
