Text For Citation: 01 Item/Group: 002 Hazard:
29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence as required by 29 CFR 1910.147(d)(1) through (d)(6): a. On or about September 11, 2020, and at times prior, in the Deck Cleaning Area of the Foam Press Conveyor Line, employees were exposed to machine hazards associated with moving parts and struck-by hazards during maintenance and cleaning activities. The employees were attempting to retrieve cleaning tools that had fallen below the line between the heated molds, without application of energy control per the company procedure. Preparation for shutdown was not performed [per the 1910.147(d)(1) requirements]. As a result, the machine was not shutdown [(d)(2)]; energy control application steps were not implemented as the machine's electrical and pneumatic energy had not been isolated [(d)(3)]; lockout/tagout devices were not applied [(d)(4)]; stored pneumatic energy was not relieved and residual thermal energy was not dissipated [(d)(5)]; and verification of the isolation and de-energization of the machine was not performed [(d)(6)].
