29 CFR 1926.502(d)(6)(i): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any object which is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself: A) Morton Buildings, Inc., conducting work at 26321 Hull Prairie Road, Perrysburg, OH 43551: On or about August 31, 2020, the employer did not ensure that employees were using the Guardian retractable lifelines in accordance with its design. Employees did not connect the Pelican hooks of the Guardian retractable lifelines to an independent anchorage point when using it as a fall protection device while installing the plywood deck in the loft of the barn at the residential property. Employees were exposed to the fall hazards of approximately 14 feet from the plywood deck of the loft to the ground below.
