29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: (a)Electric Supply Co. at 2301 East 60th Street North, Sioux Falls, SD 57104: On or about and at times prior to July 16, 2020, the exposing employer did not ensure employees were protected from falling through holes more than six feet above lower levels by a personal fall arrest system, cover, or guardrail systems erected around the holes. Employees were working on the ceiling of the clinical manufacturing suite and in close proximity to multiple pre-manufactured holes of varying size while installing armored cable conduit, electrical boxes and pulling electrical wiring. On July 16, 2020, an electrician was seriously injured after falling through a two feet wide by four feet long hole. This condition exposed employees to fall hazards of approximately 9 feet 4 inches.
