29 CFR 1910.146(g)(1): The employer did not provide training so that all employees whose work was regulated by 29 CFR 1910.146 (permit required confined spaces) acquired the understanding, knowledge, and skills necessary for the safe performance of the duties assigned under 29 CFR 1910.146: (a) Elite Labor Services, Ltd., dba Elite Staffing, Inc., at 5395 Franklin Street, Denver, CO: On and preceding 7/20/2020, the employer did not provide permit required confined space training to employees who entered a permit required confined space. Employees entered the CP Manufacturing OCCScreen machine through a small access hatch to dislodge debris collected within the spindles or discs. The screening machine utilized electricity to rotate the spindles. The employees moved within the screening machine upon the unstable surface of the spindles and discs. Gaps existed between spindles and discs that were large enough to entrap employee's feet or legs. The OCCScreen machine was a permit required confined space. This condition exposed employees to an entrapment hazard.
