29 CFR 1910.146(d)(6): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not provide at least one attendant outside the permit space into which entry was authorized for the duration of entry operations: (a) Elite Labor Services, Ltd., dba Elite Staffing, Inc., at 5395 Franklin Street, Denver, CO: On and preceding 7/20/2020, the employer did not provide at least one attendant outside the CP Manufacturing OCCScreen permit required confined space during entry operations. Employees entered the OCCScreen machine through a small access hatch to dislodge debris collected within the spindles or discs. The screening machine utilized electricity to rotate the spindles. The employees moved within the screening machine upon the unstable surface of the spindles and discs. Gaps existed between spindles and discs that were large enough to entrap employee's feet or legs. This condition exposed employees to an entrapment hazard.
