29 CFR 1910.146(c)(4): When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with 29 CFR 1910.146: (a) Elite Labor Services, Ltd., dba Elite Staffing, Inc., at 5395 Franklin Street, Denver, CO: On and preceding 7/20/2020, the employer did not develop and implement a written permit required confined space entry program. Employees entered the CP Manufacturing OCCScreen machine through a small access hatch to dislodge debris collected within the spindles or discs. The screening machine utilized electricity to rotate the spindles. The employees moved within the screening machine upon the unstable surface of the spindles and discs. Gaps existed between spindles and discs that were large enough to entrap employee's feet or legs. The OCCScreen machine was a permit required confined space. This condition exposed employees to an entrapment hazard.
