29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: (a) On or about 06/25/2020 - Large Cooler Area, employees were exposed to caught-in hazards at the Palletizer #1 and Strapex machines, when the employer did not provide locks or other hardware for Loader employees to utilize to control potentially hazardous mechanical (kinetic) energy when performing activities such as clearing jams, repositioning product, triggering the gate for the product count, cleaning up spilled product, and replacing the banding materials.
