29 CFR 1926.454(b): The employer did not have each employee who was involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question: A.) Larry Bittner dba Bittner Roofing, 3603 Peach Street, Erie, PA 16508, on or about June 17, 2020 and times prior thereto: Employees were utilizing a ladder jack scaffold system during work operations that included, but were not limited to, roofing activities and dip edge installation. Employees did not have training on the proper erection of ladder jack scaffold systems. B.) Larry Bittner dba Bittner Roofing, 3603 Peach Street, Erie, PA 16508, on or about June 17, 2020 and times prior thereto: Employees were utilizing a ladder jack scaffold system during work operations that included, but were not limited to, roofing activities and dip edge installation. Employees did not have training on the proper access setup of ladder jack scaffold systems. C.) Larry Bittner dba Bittner Roofing, 3603 Peach Street, Erie, PA 16508, on or about June 17, 2020 and times prior thereto: Employees were utilizing a roof jack scaffold system during work operations that included but were not limited to, roofing activities. Employees did not have training on the proper setup of fall protection system utilizing the roof jack scaffolding.
