29 CFR 1910.147(c)(7)(i)(A): Each authorized employee had not received training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: Training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation was not provided to employee authorized to complete service and maintenance activities at the facility. On or about February 19, 2020, an untrained employee, in the process of completing a tool change of the #4 header press; sustained a crushing injury when the energy isolating device (foot pedal) that activates a breaking mechanism to prevent the flywheel from rotating failed. The header press was not locked out prior to the start of servicing activity, and the gravitational energy remained uncontrolled.
