29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items of this section: (a) Kunzler & Company, Inc., 652 Manor Street, Lancaster, Pennsylvania - Employees performed servicing and maintenance activities, such as set up on the Upper Packing Room Line 1 and Line 2 loader and packaging machinery. The lockout/tagout program and procedures lacked the specificity needed to enable employees to safely and effectively control hazardous energy when working on particular machines or equipment, exposing the employees to hazards such as electrical, caught in, and struck by, on or about February 21, 2020, as follows: - The machine-specific procedures for the Upper Packing Room Line 1 and Line 2 loader and packaging machinery did not give the magnitude of the energy sources; - Neither the general lockout/tagout program nor the machine-specific procedures for the Upper Packing Room Line 1 and Line 2 loader and packaging machinery included provisions for temporarily removing lockout/tagout devices in order to test or position the machinery; and - Neither the general lockout/tagout program nor the machine-specific procedures for the Upper Packing Room Line 1 and Line 2 loader and packaging machinery specifically identified authorized and affected employees. Kunzler & Company, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.147(c)(4)(ii), which was contained in OSHA inspection number 1350905, citation number 1, item number 1b and was affirmed as a final order on December 4, 2018 with respect to a workplace located at 652 Manor Street, Lancaster, Pennsylvania 17603. Abatement certification and documentation is required within 10 days after the abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
