Text For Citation: 01 Item/Group: 001C Hazard:
29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and de-energization of the machine or equipment had been accomplished: (a) Upper Packing Room - Employees applied lockout/tagout to the emergency stop on Line 2's operations station to do a product changeover, which involved making changes to the loader head, as well as changing dies and plugs. The employees did not verify isolation and de-energization of the machinery prior to starting the changeover, exposing the employees to hazards such as electrical, struck by, and caught in, on or about February 21, 2020. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
