29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) Upper Packing Room - When setting up Line 1 and Line 2 loader and packaging machinery at the start of the day and after product changeovers during the day, employees did not follow the established machine-specific lockout/tagout procedures. The machine-specific lockout/tagout procedure for the loader machinery says to turn off the electrical switch at the back of the machine and apply a lock and lock the main disconnect in the electrical room. The machine-specific procedure for the packaging machinery identifies electric, compressed air, vacuum, and water as energy sources, and this procedure says to turn off and lockout the energy sources, as follows: the water at the front of the machine, the electric switch in the electrical room (which also causes the vacuum to bleed out), and the ball valve at the start of the machine. Employees doing set up on Line 1 and Line 2 only applied lockout/tagout to the emergency stop at each line's operations station and did not lockout/tagout the other energy sources, exposing the employees to hazards such as electrical, struck by, and caught in, on or about February 21, 2020. (b) Upper Packing Room - When setting up Line 1 and Line 2 loader and packaging machinery at the start of the day and after product changeovers during the day, employees applied lockout/tagout to the emergency stop at each line's operations station, which constitutes control circuitry, rather than applying lockout/tagout to an energy isolating device, such as an electrical disconnect switch, exposing the employees to hazards, such as electrical, struck by, and caught in, associated with unexpected energization or start up, on or about February 21, 2020. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
