1910.147(c)(7)(iii)(B) Additional retraining shall also be conducted whenever a periodic inspection under paragraph (c)(6) of this section reveals, or whenever the employer has reason to believe that there are deviations from or inadequacies in the employee's knowledge or use of the energy control procedures. (a) The J.M. Smuckers Company at 2900 Peak Avenue, Longmont, CO 80504: On and before February 7, 2020 the employer did not retrain affected employees on the machine specific energy control procedures for the control of potentially hazardous energy for the conveyor dough rolling equipment when it had reason to believe there were deviations from or inadequacies in the employee's knowledge or use of the energy control procedures. An affected employee used an equipment manufacturer's maintenance access key to open and remove a protective panel on a section of the conveyor equipment. The employee sustained serious hand and arm injuries when her hand and arm were pulled into the machine between two rollers. This condition exposed employees to crushing and amputation hazards. (b) The J.M. Smuckers Company at 2900 Peak Avenue, Longmont, CO 80504: On and before February 7, 2020 the employer did not retrain affected employees on the machine specific energy control procedures for the control of potentially hazardous energy for the conveyor dough rolling equipment on the Bread Line. Employees and operators only press the E-stop on conveyor equipment before they place their hands in the danger zone of equipment that is not properly de-energized. This condition exposed employees to crushing and amputation hazards.
