29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels by personal fall arrest systems, covers, or guardrail systems erected around such holes: a. By the elevator shaft on the third floor of a new residential construction located at Lot 6 Vermillion Way, Santa Rosa Beach, Florida, 32459: On or about January 28, 2020, and at times prior, the employer exposed employees to an over 25 foot fall hazard, in that an employee was installing light fixtures near an unprotected (by fall arrest systems, covers, or guardrail systems) elevator shaft ultimately falling through it.
