29 CFR 1910.28(b)(3)(i): The employer did not ensure that each employee was protected from falling through any hole(s) that were 4 feet (1.2 m) or more above a lower level: USS Anchorage, Ballast Tank 7-158-1-W: Employees performed ship repair operations, such as but not limited to MIG welding. The walking-working surface had holes greater than 4 feet above the lower level; the holes were covered by plywood sheets that were not secured to prevent shifting. The employer did not ensure the holes were protected and employees were exposed to fall hazards.
