29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment: a) Machine Shop - The employer performed an inadequate workplace hazard assessment in that employees were permitted to don material handling and nitrile gloves while handling and building parts and tools, thereby exposing employees to caught-in and entanglement hazards while working next to rotating parts. While hand polishing a work piece inside a Haas CNC Lathe, an employee sustained an amputation injury when one of the gloves became entangled in the rotating parts, on or about January 22, 2020. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
