29 CFR 1910.132(b): Where employees provided their own protective equipment, the employer did not ensure its adequacy, including proper maintenance, and sanitation of the equipment: a) Haas CNC Lathe, Model DS-30Y - The employer did not assure that employee-owned equipment was adequate as required, in that employees were not prohibited from donning gloves which created an entanglement hazard. While hand polishing a work piece inside a Haas CNC Lathe, an employee sustained an amputation injury when one of the gloves became entangled in the rotating parts, on or about January 22, 2020. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
