29 CFR 1910.212(a)(3)(ii): Point of operation of machinery were not guarded or guards were not designed and constructed to prevent employees from having any part of their body in the danger zone during operating cycles: (a)Hydro Extrusion USA, LLC at 2500 Alumax Road, Yankton, SD 57078: On or about and at times prior to January 23, 2020, the employer did not ensure employees were protected from the point of operation while operating the horizontal hydraulic press located in the maintenance shop without an effective machine guard. This condition exposed employees to an amputation hazard. Abatement Note: Examples of acceptable machine guards may include barrier guards, two hand tripping devices, and electronic light curtains. Additionally, use of special hand tools to keep hands away from the point of operation is acceptable regarding 29 CFR 1910.212(a)(3(iii). Abatement Note: Safeguarding by maintaining a "safe distance" is acceptable, during normal production operation, if the operating employee does not approach closer than necessary and in no case, closer than 4 inches (10.16 centimeters), to the point of operation as required under CPL 2-1.25, Guidelines for Point of Operation Guarding of Power Press Brakes paragraph and ANSI B11.3-1982, Safety Requirements for Power Press Brakes. One feasible and acceptable abatement method to correct this hazard is to ensure that each employee who works on the power press brakes receives documented training, and supervision to ensure that operating power press brakes safely by maintaining a safe distance from the point of operation.
