Text For Citation: 01 Item/Group: 001B Hazard:
29 CFR 1910.333(b)(2): While an employee was exposed to contact with parts of fixed electric equipment or circuits which were deenergized, the circuits energizing the parts were not locked out or tagged or both in accordance with the requirements of this paragraph (b) of 29 CFR 1910.333(b)(2): The employer did not ensure all electrical equipment circuits were deenergized with a lock, a tag or both prior to employees working on or near the live or energized equipment. Employees were exposed to electric shock and arc-flash hazards: On or about January 10, 2020, and at times prior thereto, employees worked on a switch gear and relied on another company, Oncor Delivery, to deenergize or apply lockout/tagout devices which did not ensure employees were protected from all energized parts, and On or about January 11, 2020, and at times prior thereto, the employer did not ensure employees deenergized the equipment prior to opening a switch gear cabinet which exposed employees to an arch flash incident, and On or about January 11, 2020, and at times prior thereto, the employer did not ensure employees deenergized the equipment when a crew responded after an arch flash incident to finish repairing the switch gear.
