29 CFR 1910.147(c)(4): Energy control procedure. 29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: 29 CFR 1910.147(c)(4)(ii)(B): Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; a. On or about October 15, 2019, the employer's energy control procedures for Weld department Tunnel Lines did not include specific procedural steps to address potential stored mechanical energy associated with jig cart set-up activities. Employees performed required duties covered by this part that involved stored mechanical energy exposure when jig cart suspension occurred due to positioning pin misalignment during the activity. On October 7, 2019, an employee suffered a partial finger amputation related to the performance of the jig cart set-up task on Tunnel Line 1 at station 10.
