29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when an employee was engaged in the activities covered by this section. On or about September 25, 2019, at the KIK Custom Products Processor Tank Farm in Houston, Texas; the employer failed to develop specific procedural steps for isolating, blocking, and securing machines or equipment to control hazardous energy when leak repair was performed on the 16% concentrated bleach piping leading to the Unfiltered Tanks #1-2 & Bodyfeed Tank.
