29 CFR 1910.147(d): 29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) On or about September 16, 2019, employees were exposed to struck-by and crushed-by hazards from the lift table of the stacker on the PLC Inotech sorting and stacking machine when attempting to remove a sticker that lodged under the table holding it up. The employer did not ensure employees implemented energy control application steps prior to allowing them to engage in servicing and maintenance tasks. The equipment was not deenergized prior to employees attempting to remove the sticker and an employee suffered an amputation when the lift table fell unexpectedly. As a result, the applicable energy control elements, involving preparation for shutdown [1910.147(d)(1)], machine shutdown [1910.147(d)(2)], machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.
