Text For Citation: 01 Item/Group: 003 Hazard:
29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: a.) Benjamin Steel Company, Inc. located at 3111 St. Johns Rd. Lima, Ohio; On or about September 25, 2019, the employer did not ensure that employees were adequately trained on the company's lockout program for the HFA-400W AMADA horizontal metal cutting band saw located in the metal process and service area. Employees did not lock out and tag out the electrical panel while clearing trapped metal rounds material or during blade changeout activities on equipment including, the HFA-400W AMADA horizontal metal cutting band saw. Employees were exposed to caught in hazards such as laceration and amputations in that the HFA-400W AMADA horizontal metal cutting band saw remained energized as employees frequently conducted metal rounds material clearing and during blade changeout.
