Text For Citation: 01 Item/Group: 001A Hazard:
29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a. On or about September 18, 2019, located in the sawmill at Albany Bandmill, employees were exposed to machine hazards associated with moving parts of a model number 8ft, serial number 259RH, bandsaw and its hydraulic guard when performing a blade change. The employer did not implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards. On September 18, 2019, an employees hand was inadvertently placed too close to a pinch point when a hydraulically-actuated barrier guard was closing during a blade change activity on a bandsaw resulting in a fingertip amputation.
