29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On or about 08/19/2019 - Damage Control Area, employees were exposed to caught-between hazards when a procedure was not utilized to control potentially hazardous energy such as, but not limited, to the unexpected ram movement of a PRT Self-Contained Compactor when employees entered the loading hopper to clear a material jam.
