29 CFR 1910.147(e)(3): The specific procedure, for when the authorized employee who applied the lockout or tagout device is not available to remove it, did not include at least the following elements of 29 CFR 1910.147(e)(3)(i) through (e)(3)(iii): a) Penn Highlands Elk, 763 Johnsonburg Rd. in Saint Marys, PA. On or about August 15, 2019: In the written Lockout/Tagout Program, the procedure for the removal of locks left on equipment after work is complete did not include steps to ensure: 1. All reasonable efforts were made to contact the authorized employee to inform him/her that his/her lockout or tagout device has been removed, and 2. The authorized employee has knowledge before he/she resumes work at that facility that their lockout/tagout device was removed. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
