29 CFR 1910.178(l)(3)(ii)(E): Powered industrial truck operators did not receive initial training on narrow aisles and other restricted places where the vehicle would be operated: (a) On or about August 12, 2019 - Warehousing/production areas, employees were exposed to struck-by hazards when standup powered industrial truck training did not include information specific to narrow/restricted places such as, but not limited to, the narrow PMZ gate and the aisle in front of the brazing department. Training such as, but not limited to, how to approach the narrow site, how to enter the narrow site, and whether entry should be made with forks leading or trailing was not provided.
