29 CFR 1910.30(c)(3): When inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee no longer has the requisite understanding or skill necessary to use equipment or perform the job safely. On or about August 5, 2019, the employer did not ensure all employees' knowledge of fall protection systems and equipment were appropriate to use the proper equipment to perform the job safely. Employees were not knowledgeable in the use of rebar snap hooks, also known as pelican hooks, inappropriate anchorage connections, inappropriate anchorage strength, anchoring below the dorsal D-ring, and confusing the twin-leg energy-absorbing and self-retracting devices.
