29 CFR 1910.28(b)(6)(i): Each employee less than 4 feet (1.2 m) above dangerous equipment was not protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system, unless the equipment is covered or guarded to eliminate the hazard: a. On or about August 5, 2019, the employer did not ensure employees working at the Finishing Division Tandem Mill Bander Machine were protected from falls into or onto the tandem mill coil transfer conveyor machinery and coil pit on the entry side of the tandem mill coil bander machine. b. On or about August 5, 2019, the employer did not ensure employees working at the Finishing Division Tandem Mill Bander Machine were protected from falls into or onto the ADS line coil conveyor machinery on the exit side of the tandem mill coil bander machine. c. On or about August 5, 2019, the employer did not ensure employees working at the Finishing Division Tandem Mill Bander Machine were protected from falls into or onto the coil conveyor machinery when crossing the coil conveyor from the bander control booth to the bander machine.
