Text For Citation: 01 Item/Group: 002 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees when they were exposed to struck-by hazards during leak testing of a metal container : a) On or about 08/07/19 in the Welding department; employer wrote a leak testing procedure to identify and repair leaks on an assembly housing (welded metal container 40" x 16.5" x 15" - 4 cubic feet). Employer provided unregulated plant air (118 psig.) to pressurize the metal container, did not provide with any pressure relief device for the container during the leak testing and provided an oil based liquid for lubrication of bolts used to secure the container covers. During a leak testing of an assembly housing (welded container), one of the six covers, bolted to the housing to seal the openings, unexpectedly and forcefully detached striking an employee causing serious injury ( severe laceration to arm and abdominal contusions).
