29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Nox US, LLC - Fostoria, Ohio: The employer failed to implement and apply energy control application steps for authorized employees that followed the procedures and sequence set forth in the regulation to control hazardous energy sources on or about July 23, 2021, exposing them to crushing, burn and amputation injuries on Lamination Line 2.
