29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) K2D, Inc. dba Colorado Premium Foods at 5120 Race Court in Denver, CO 80216: On or before July 2, 2019, the employer did not ensure that the point of operation on the Weiler Frozen Block Grinder was effectively guarded, in that a metal shroud only covers the top half of the output section of the equipment. On June 27, 2019 an employee operating the equipment sustained an amputation injury. This condition exposed employees to amputation and other serious hazards. (b) K2D, Inc. dba Colorado Premium Foods at 5120 Race Court in Denver, CO 80216: On or before July 2, 2019, the employer did not ensure that the point of operation on the VC-999 Sealer was effectively guarded. An opening between the conveyor and the bottom section of the clear plastic cover was approximately 4.75 inches and was approximately 14 inches from the cutting knives. In reference to table O-10 in Subpart O, the maximum opening is 1 and 7/8 inches. This condition exposed employees to amputation and other serious hazards.
