IbakeFoods, LLC, 256 Baker Street, Rimersburg, PA 16248, maintenance shop, on or about June 12, 2019 and times prior thereto: A flexible cord was spliced into an electrical breaker box in the maintenance shop and was ran to a stretch wrapping machine in the wareroom. B.) IbakeFoods, LLC, 256 Baker Street, Rimersburg, PA 16248, sanitation room, on or about June 12, 2019 and times prior thereto: The electrical cover plate for an electrical outlet box was not securely affixed to the box.
