IbakeFoods, LLC, 256 Baker Street, Rimersburg, PA 16248, on or about June 12, 2019 and times prior thereto: Employees were provided with, and permitted to use, portable dry chemical fire extinguishers. The employer did not visually inspected the portable fire extinguishers at least monthly.
